Western Australian industry saw a nightmarish day yesterday with the deaths of three workers in a matter of hours, one of whom was killed at the Alcoa Refinery in Kwinana.
The man died after he fell 10 metres from scaffolding, prompting a full investigation by Alcoa and Worksafe WA, with investigators on the scene on the same day.
Two other workers died yesterday in an unrelated incident, after they were crushed by a falling concrete slab at an East Perth construction site.
A statement from Alcoa said the company was deeply saddened by the death.
"No other employees or contractors were injured in the accident," the statement read.
"Alcoa is working closely with the relevant authorities and providing information and support to its workforce."
